Program Summary
One Day at a Time (ODAAT) operates a Police Assisted Diversion (PAD) program for the 22nd District. PAD is a program offered to divert participants who were stopped for a variety of criminal offenses. These offenses include: low-level, non violent drug, prostitution, and retail theft offenses within the designated district and during program operating hours. The program works to divert participants away from the criminal justice system and towards supportive, peer-based social services that are customized to the participants’ needs.
PAD is a collaboration between police officers, service providers, and community residents, and takes a health-centered approach to law enforcement. PAD at ODAAT, works with engaged participants to address issues such as access to basic needs, access to treatment services, appropriate medical care, and the development and achievement of short and long-term goals. PAD services are voluntary, free to the participant, and may continue for as long as the participant decides to remain engaged in program services.
The team consists of an Assistant Director (MSW), two Case Managers and two peer support. All collaborate with the referring officer and participant to provide a comprehensive service plan for individuals geared to reduce the risk of criminal activity. The PAD team members will approach each participant with a holistic community approach and utilize a variety of tools including assessments, evaluations, and intake procedures.
Position Summary
The PAD Case Manager (PCM) is a caring and compassionate individual who performs professional and administrative duties to screen, assess, facilitate and coordinate a new unique diversion program that provides alternative resources for individuals to avoid placement in jail and/or future police related encounters. PCM will provide biopsychosocial assessment, service care plans, inter-agency collaboration and referrals. PCM will conduct outreach and refer for adjunct services such as social services, mental health or non-treatment related services. PCM will provide knowledge of available resources to the PAD team to ensure diversion participants are receiving maximum benefits of the PAD program.
Key Duties & Responsibilities:
- Coordinates the intake of participants when police department brings participant for service
- Performs a variety of direct clinical, diagnostic, trauma focused engagement and assessment, and outreach services.
- Conducts case management duties regarding service plan, keeps case progress notes, and related matters to PAD enrollment.
- May assess social, emotional and economic causes of problem behavior and mental or emotional help; in doing so develop and or modify goals and objectives for service plans.
- Maintain a client file for auditing and to ensure compliance.
- Ensures compliance with related federal, state and local regulations, as well as ODAAT/UAC and county policies and procedures.
- Assists with program planning, implementation and evaluation of activities.
- Participates in community outreach and public relations activities.
- Work cooperatively and collaboratively with business owners, police, and
- concerned citizens to serve the needs of community and participants.
- Assist with public, media and internet outreach with other staff and program volunteers to support intake into the programs and attendance at sponsored events.
- Assist with planning and supporting socialization and outreach events.
- Attend all mandated training activities.
- Provide programming under the Harm/Risk reduction and Trauma-informed service spectrum and ODAAT services to high risk individuals.
- Ability to organize time, prioritize assignments, including the ability to work flexible hours.
- Actively support executives with client and partner collaboration.

Education, Knowledge, Skills & Abilities (Minimum Qualifications):
- Possess ability to understand various policing and community operations and procedures
- Able to maintain a professional, customer service-oriented attitude at all times with strong interpersonal skills
- Ability to meet face-to-face with individuals of diverse backgrounds and disabilities, including physical, mental, substance abuse, HIV/AIDS, etc.
- Proficient with Microsoft Office software, including Excel and Outlook
- Demonstrated written and verbal communication skills including typing and writing
- Must have the ability to work independently and as a team member
- Must take initiative and ability to problem solve
- Ability to learn and apply written policies and procedures related to work
- Able to set and observe appropriate boundaries with clients
- Possess well-developed decision making skills and attention to detail
- Able to multitask and remain calm in demanding and unpredictable situations
- Possess excellent time management skills and the ability to work with minimum supervision
- Ability to transport individuals as needed, and travel to various sites and locations
- Able to meet a flexible work schedule including holidays, evenings and weekends
- Available on-call for emergencies by telephone and/or cell phone; and
- Performs any other duties assigned
- Bachelor’s Degree in Social Work or related field, preferred.
- 1+ years of prior outreach and community work experience in the field and/or related field preferred.
- Proficient in Microsoft Office Suite software.
- Ability to manage time and respond to deadlines.
- Strong writing, typing, reading, listening and speaking communications skills.
- Able to develop rapport, provide information, and refer with sensitivity to cultural issues.
- Bilingual a plus.

PHYSICAL REQUIREMENTS
- Able to lift/move weight up to 50lbs.
- Vision adequate to perform the responsibilities and functions of the job efficiently.
- Must be able to sit for 2 or more hours per day
- Must be able to stand for 2 or more hours per day
WORK ENVIRONMENT
The employee will work in an office and public, service-delivery environment. Hectic situations can occur in the public area of the facility characteristic to working with the homeless community and the related concerns and situations related to homeless conditions. Potential exists for volatile and unpredictable situations to occur. May experience exposure to strong odors such as soiled clothes and poor hygiene.
